who utilize an eye care practice software system called RevolutionEHR
to give their patients access to certain personal eye care provider
information online. RevolutionPHR is available through a secure
internet connection at www.revolutionphr.com. This is a voluntary
service, offered by your eye care provider, and only available to
individuals who receive eye care services from eye care providers who
use RevolutionEHR.

The following terms and conditions govern all use of the RevolutionPHR
site and all content, services and products available at or through the
site, (taken together, the “Site”). The Site is owned and operated by
RevolutionEHR, a product of Health Innovation Technologies, Inc. The
Site is offered subject to your acceptance without modification of all of
the terms and conditions contained herein and all other operating
rules, policies and procedures that may be published from time to time
on this Site by your eye care provider or its agents.

Your information will not be sold to outside vendors and will be used
only for the purpose you intend. Your eye care provider and is agents
are not responsible for any activity of others that results in your
information delivered through the website to others.

provider. It is extremely important that you keep the username and

RevolutionPHR should never be used for emergencies or urgent needs.
If you are experiencing a life threatening emergency you should call 911.

RevolutionPHR may be revised, changed or updated periodically.
RevolutionPHR may not always be available due to system
